Insurance verification / Demographics and claim entry

First step in creating the claim is obtaining demographics, insurance information and icd/cpt codes. After collecting, we verify insurance information, such as claim number and correct mailing address for billing submission, necessary to correctly complete data entry process and claim creation.

Accounts Receivable Follow-Up

Once Health Insurance Claims are submitted to various insurance companies and enough time has passed allowing insurance companies to properly process claims, we are able to start obtaining information about bill status. We can use four methods:

  • Online Bill Status Follow-Up. Various insurance companies are giving the opportunity to review claim status and obtain Explanation of Benefits through their web sites and internet payer portals.
  • Automated Bill Status Follow-Up. Some companies have an Interactive Voice Response system (IVR) that provides status of processed bills.
  • Bill Review Department (Insurance Company Representative). When in need of detailed explanation, we are, however, able to reach bill review department, or in some cases, claims adjuster.
  • Written requests. Most insurance companies have email addresses allowing us to request any kind of denial to be emailed to us directly.

Denials Management

Once denials are obtained, extra effort is needed to overturn decision from "denied" to "reprocessed and paid". Depending on denial reason there are multiple options:

  • Claim Correction. If claim has been denied due to a technical error, it has to be modified and resubmitted as a corrected claim to insurance company.
  • Documentation missing. If claim has been denied due to a documentation missing, it has to be resubmitted as a corrected claim with missing documents added.
  • Reimbursement guidelines, state fee schedules and similar. If claim has been denied or underpaid, appeal has to be sent with appropriate objections.
  • Patient's responsibility. If claim has been denied due to In-Network deductibles or non-covered benefits per the insurance plan or policy and all efforts have been exhausted, the final bill has to be sent to the patient for payment collection.
